Every three years AIDS Alabama completes a state wide Needs Assessment Survey of persons living with HIV in the State of Alabama.  Any HIV-positive person is eligible and encouraged to complete the survey.  The information collected through the questions you answer is used by all AIDS Service Organizations in the state to make decisions on how to best meet the needs of the client.  The information is also used to determine what issues or needs might be emerging so we can develop a plan to meet those needs before they become a problem.  The data collected also helps us to plan for the future.

Each HIV-positive person completing the Needs Assessment Survey will receive a $15.00 Walmart gift card as our thanks to you for assisting with this important project.  The questions cover a wide range of topics from the general state of your health to your current treatment plan, what medications you take, activity levels and general information about how you live with HIV.  We also ask for your most recent CD4 and Viral Load numbers.  Please contact one of the following staff persons to arrange to complete the survey at your convenience.
